BMI gets treated like a verdict on someone's health, which is a bigger claim than the number was ever designed to support. Here's what it actually measures, where it holds up, and where it genuinely falls apart.
What BMI actually is
Body Mass Index is simply your weight divided by the square of your height. That's it — there's no measurement of body fat, muscle mass, bone density, or where your weight is distributed. It was developed in the 1830s by a Belgian mathematician studying population averages, not individual health — it was never intended as a personal diagnostic tool in the first place.
Where BMI is genuinely useful
At a population level, BMI correlates reasonably well with health outcomes when you're looking at large groups of people. Public health researchers use it to track obesity trends across populations, compare health statistics between countries, or screen large groups quickly and cheaply, since it only requires a scale and a measuring tape. For this kind of broad, statistical use, it does its job fine.
Where it falls apart for individuals
The problem is BMI can't distinguish between muscle and fat. A muscular athlete and a sedentary person can have the exact same BMI while having very different body compositions and health profiles — the formula has no way to tell them apart. This is the most common criticism, and it's a real one: plenty of visibly fit, muscular people get flagged as "overweight" or even "obese" by BMI alone.
It also doesn't account for where fat is stored. Fat around the abdomen is associated with higher health risk than fat elsewhere, but two people with identical BMI can have very different fat distribution — and BMI has no way to capture that difference.
Age and sex differences matter too. The same BMI can mean something different for a 25-year-old versus a 65-year-old, or for a man versus a woman, since body composition naturally differs across these groups — but the standard BMI formula applies one set of categories to everyone.
So should you ignore it?
Not entirely — it's still a reasonable first-pass screening number, especially if you fall well outside the "normal" range in either direction. But it's genuinely just one data point, not a diagnosis. If your BMI is borderline or seems off given how you actually feel and perform physically, it's far more useful as a conversation starter with a doctor than as a conclusion on its own. Other measurements — waist circumference, body fat percentage, or simply how you're actually doing physically — often tell a more complete story than a single ratio of weight to height ever could.
